If your insurance plan doesn't approve INVOKANA®, INVOKAMET® XR, or INVOKAMET®:

Sometimes, your insurance company may deny coverage of a medicine your doctor has prescribed for you. For example, your insurance plan might not cover a medicine because it is not on their approved prescription drug list (Formulary) or they may have other rules or policies that cause a denial of coverage. If this happens, you and your doctor can request that your insurance company cover the medicine through an exception process. You can ask your doctor to complete a Letter of Medical Necessity or Letter of Exception depending on the reason the medicine isn't covered for you. Samples of both letter types are available for your doctor to download at JNJwithMe.com.
